Depending on your care needs, we may process identity and address details, gender, contact details, health information, insurer and healthcare-provider information, appointment details and payment details.

We process data for patient records, appointments, treatment, preferences and requested services. Legal bases include performance of the treatment agreement and legitimate interests under Article 6(1)(b) and (f) GDPR.

We share personal data only with your consent, where necessary for the treatment agreement or where required by law.

Medical records are retained for the statutory 20-year period under Dutch law. Financial records are retained for 7 years.

The practice describes appropriate security measures and work in accordance with NEN 7510 for storing, processing and protecting patient data.

You may request access, rectification, deletion, transfer or restriction and object to processing. Contact info@tandartspraktijkkampveld.nl or 015 212 15 80. The practice aims to respond within 14 working days.

The contact form asks for your name, telephone number, email address, subject and message. The registration form additionally asks for your title, date of birth, address, postcode, city, whether you are registering yourself or a child and, if applicable, the child's name and parent or guardian. We use these details only to answer your question or process your registration. Submissions are forwarded to the practice by email and stored as a secured backup in the website's secure environment and in a restricted Google Sheet. We do not collect citizen service numbers, medical or insurance details via the website and do not use form content for analytics or marketing. Retention period: [to be confirmed by the practice].
